In the Information Technology era, along with the development of science and technology, all fields of human life are digitized at a rapid rate. In which, medical, especially community health care, is one of the industries with the fastest speed of modernization. That development has helped detect, diagnose, and treat many dangerous diseases in a timely manner. Not only that, but this also helps to ensure the health of each of us and contributes to increasing the quality of human living environment. In this project, our team wants to come up with a solution to contribute a part of our efforts to build on the overall development of community healthcare. Firstly, our solution provides a digital storage solution for personal health information and prescription, alternative to the traditional paper version. Secondly, the solution needs to ensure the confidentiality of "sensitive" information in the medical field. To meet the above requirements, we use a database system to store information about prescriptions as well as medical information and medical history of users. After that, we build a backend system so that users (including medical staff, patients, etc.) can interact with those data through Application Programming Interfaces (APIs). Finally, we create website and mobile applications that implements those APIs and provide the Graphic User Interface (GUI) solution for our system. Our system uses cryptographic tokens to ensure security, as well as a session password QR code to communicate between web and mobile applications. The contribute of our solution provide both an efficient digital solution for personal health information and a new security idea for QR codes, a technology that is being used more widely, especially since the Covid-19 pandemic. Moreover, this project also helps to demonstrate the optimization of a digital processing system compared to using traditional paper records
